This Order Form is For the Indianapolis, Indiana & Vicinity franchise ONLY.
Our service area covers from Greenwood, IN to Kokomo, IN and from
Anderson, IN to Danville, IN and all of Indianapolis.

Prices subject to change. Taxes and delivery charge not included in price
Delivery is normally within 1 to 2 business days excluding holidays.
Delivery charges range from $5 to $10.

For an extra surcharge we will sometimes deliver outside of our
normal delivery area. To see if we can cover a specific area, it is
best to call our store during normal business hours to find out
if we can service your area and what an appropriate fee would be.
